个人独资企业设立登记申请书
企业名称
1、 在签署文件和填表前，申请人应当认真阅读《中华人民共和国
个人独资企业法》、《个人独资企业登记管理办法》和本申请书，并确知
其享有的权利和应承担的义务。

注：(1)登记机关收到申请人提交的文件材料后进行查验，在“备注”栏中填写材
料份数。
(2)企业住所证明：投资人自有的住所，应当提交房管部门出具的产权证明； 租用他人的场所，应当提交租赁协议和出租人的产权证明。企业住所不 是其生产经营场所的，投资人除了提交企业住所证明外，还应当提交生产 经营场的证明。投资人自有的经营场所，应当提交房管部门出具的产权 证明；租用他人的场所，应当提交租赁协议。
(3)申请人是指向登记机关提出设立登记申请的人。申请人是投资人的，应 提交投资人的身份证明；申请人是投资人委托代理人的，应提交投资人的 委托书和代理人的身份证明或资格证明。

注：(1)“企业住所”应填写所在市、县、乡(镇)及村、街道门牌号码。
(2)出资额是投资人以货币出资的数额，以及采取实物、土地使用权、知识产 权或者其他财产权利出资的作价数额，投资人申报的出资额应当与企业 的生产经营规模相适应。
(3)“出资方式”栏中，在选择项的序号上划“√”。以家庭共有财产作为个人 出资的，家庭成员应当签名。
(4)“从业人员数”应填写企业拟聘用从业人员的数量。 (5)出资额、出资方式和从业人员数由投资人申报。
