出国留学申请表是在3月份CSC开放报名的时候在网上填写的。
难道今年情况变了？还是你们学校搞错了？。。。
是三月二十几号在网上填写 但是我们学校三月初就是初选 之后才有资格在网上填写 初选时候就要交这几个表格

关于准备2011 年国家留学基金资助出国留学申请材料的说明 (学生类申请人用) 1、国家留学基金管理委员会出国留学申请表（学生类） 申请人需先登录网上报名系统，并按要求如实填写网上申请表；在填写完申请表并确 认无误后，可按系统提示完成网上提交并打印。申请表中的有关栏目应视实际情况和项目要 求进行填写，如无相关情况可不填（如工作经历）。申请人提交的书面申请表应与网上报名 信息内容一致。网上申请表正式提交后不能再修改信息（如留学期限、留学国别等）。申请 人需在每份申请材料“申请人保证”栏中签名。
凡来自国家建设高水平大学公派研究生项目60 所签约高校（不含攻读硕士学位人员） 及未签约“211”工程院校申请赴国外攻读博士学位人员，应统一申报国家建设高水平大学公 派研究生项目，其他人员申报国家公派专项研究生奖学金项目。凡申请攻读硕士学位人员（含 60 所签约高校）应申报国家公派专项研究生奖学金项目。如：华中农业大学（未签约“211” 院校）申请赴国外攻读博士学位人员应申报国家建设高水平大学公派研究生项目；北京大学 （签约院校）申请瑞典皇家工学院硕士生项目人员应申报国家公派专项研究生奖学金项目。
2、单位推荐意见表 单位推荐意见表在申请人打印申请表时由网上报名系统自动生成（申请人在网上报名 阶段此表不在报名系统中显示）。推荐意见应由申请人所在部门（院、系、所等）针对每位 申请人填写（内容需注明被推荐的应届本科毕业生在年级、班级学习成绩排名）。上级批准 意见由所在单位负责选拔工作的主管部门在认真核对申请人所填信息后填写，应加盖推荐单 位公章（司局级以上或学校公章)。

5、身份证复印件 请申请人将身份证正反面（个人信息、证件有效期和发证机关）同时复印在同一张A4 纸上。
6、正式邀请信复印件或入学通知复印件 （1）申请人应提交正式邀请信复印件/入学通知复印件。邀请信复印件/入学通知复印 件应使用拟留学院校（单位）专用信纸打印，并由主管部门负责人/导师签字。
攻读博士学位研究生申请人，如因拟留学院校（单位）行政审批手续规定限制，在申 请截止时间前无法出具正式入学通知，则须出具使用拟留学院校（单位）专用信纸打印并由 对方主管部门负责人/导师签字的明确意向入学通知。
（2）攻读博士学位研究生申请人提交的入学通知，应为无条件入学通知（unconditional offer），但不包括如下条件： a．入学通知在申请人取得国家留学基金资助后方可生效； b．入学通知在申请人提供本科毕业/硕士毕业证书后方可生效； c．入学通知明确申请人在拟留学院校/单位须完成硕士课程后可继续攻读博士学位（申 请硕博连读申请人）。
（3）邀请信/入学通知中应包含以下内容： a．申请人基本信息：申请人姓名、出生日期、国内院校等。
b．留学身份：攻读博士学位研究生或联合培养博士生。
c．留学时间：应明确起止年月。
d．国外指导教师信息。
e．留学专业或受邀人拟在国外从事主要学习/研究工作。
f．所需费用及数额：例如，须交纳的学费、注册费等有关费用名称和数额。
g．联合培养博士生申请人可在邀请信中注明被邀请人外语水平符合接受方要求，否则 应单独出具外方认可申请人外语水平的证明。
h．外方负责人签字与联系方式。

翻译件应由国内推选单位加盖审核部门公章。
（5）如申请的留学基金委与国外高校/机构合作项目对邀请信/入学通知有特殊要求， 则根据具体项目规定执行。
7、收取学费明细表、免学费或获得学费资助证明复印件或有关学习费用证明复印件（申 请学费资助人员需提交） 如在提交的邀请信复印件/入学通知复印件中未注明留学所需费用及来源，申请人须另 行提交收取学费明细表复印件、免学费或获得学费资助证明复印件或有关学习费用证明复印 a．国外拟留学院校（单位）不收取学费/其他费用（如注册费等）的证明（部分不收学费国家可免交，如德国、法国等）。
b．国外拟留学院校（单位）出具的需要收取学费/其他费用的明细表复印件和同意提 供学费/其他费用的奖学金证明复印件。所提供学费/其他费用的奖学金应可支付明细表中注 明需支付学费/其他费用的总和。
c．国外拟留学院校（单位）出具的需要收取学费/其他费用明细表复印件和同意提供 Research Assistant 或Teaching Assistant 职位，且工资总计足以支付学费/其他费用（如 注册费等）的证明。
所有奖学金证明应使用拟留学院校（单位）专用信纸打印，并有主管部门负责人签字， 不接受电子邮件或无签字的奖学金证明。
8、成绩单复印件 提供成绩单应包括本科、硕士（如有）、博士（如有）学习阶段，直至最近一学期的 成绩。成绩单应由就读单位教务处、研究生院或有关学生管理部门开具。
9、最高学历/学位证书复印件 应届本科毕业生无需提供。硕博连读生、直博生申请人应提供大学本科学历、学位证 书复印件。
10、在籍证明 申请人的学籍证明应由申请人所在单位学籍管理部门出具，说明申请人目前的学籍情 况，内容包括：被推荐人姓名，性别，出生日期，所在院系，所在年级（是否进入博士学习 阶段），进入博士阶段学习的时间和目前所在年级。学籍证明应使用带有推选单位抬头的信 函纸打印并加盖主管部门公章。

Given someproducts specificprofessional restrictions 该项申请材料只适用于联合培养博士研究生申请人。如其无法在既定毕业时间内完成 所申请的留学计划，则需要推选单位的研究生院/部出具同意申请人推迟答辩、毕业的书面 证明，内容包括：被推荐人姓名，性别，出生日期，所在院系，目前在学年级，原定毕业时 间，申请出国留学时间，推选单位关于同意其推迟答辩、毕业的意见。推迟答辩、毕业证明 应使用带有推选单位抬头的信函纸打印并加盖主管部门公章。
12、外语水平证明复印件 攻读博士学位研究生申请人应提交符合申请留学院校(单位)入学要求的有效外语考试 成绩单复印件，如IELTS、TOEFL 成绩单。
联合培养博士研究生申请人应提交有效外语考试成绩单复印件（如IELTS，TOEFL， WSK 等）或推选单位（国内学校）出具的外语考试（核）证明。
13、联合培养计划或学习计划（外文） 联合培养博士研究生申请时应提交英文联合培养计划（1000 字以上），并由中外双方 导师签字。联合培养计划如为英语以外语种书写，需另提供经国内推选单位审核的中文翻译 件（需加盖审核部门公章）。
攻读博士学位研究生申请时应提交英文学习计划（1000 字以上），并由外方导师签字。
如申请人拟在国外进行硕博连读，暂时无法确定导师，则只需国内推选单位审核并签字。学 习计划如为英语以外语种书写，需另行提供经国内推选单位审核的中文翻译件（需加盖审核 部门公章）。
联合培养计划/学习计划应包括以下主要内容： a．博士研究课题名称； b．科研课题背景介绍； c．申请人国内科研准备工作概述； d．出国学习预期目标； e．科研方法； f．科研工作时间安排； g．回国后续工作介绍。
14、主要科研成果 主要包括申请参与科研及学术论文发表情况（应列出作者排名）。切勿提交论文、著 作全文，提供论文、著作概述即可(每项成果简介最多不超过两页)。

16、获奖证书复印件 申请人仅需提供最近5 年内获奖级别最高的证书复印件，最多不超过3 17、国际旅行健康证书复印件（受理机构留存，不提交留学基金委）申请人需前往当地出入境检验检疫机构进行体检，并办理《国际旅行健康证书》。
